Terms and Conditions
1. Accessing or using this site and the information therein constitutes acceptance of the Terms and Conditions below. Oral representations or agreements not set forth in this agreement are not binding. This agreement supersedes any preceding agreement between the parties relating to the services of this site.

2. The purpose of this site is to provide an additional medium to review the status of submitted claims, and the verification of member eligibility. The accuracy, authenticity, security, and confidentiality of the information submitted to MagnaCare is the sole responsibility of the provider submitting the information.

3. By use of this site you agree to the following:

. that you shall provide access only to those individuals (employees and associates) who need it to perform their jobs and on your behalf.

. that you and your associates and employees understand that the information, viewable and printable online within the application, shall be handled in the same confidential manner that sensitive medical and personal information is handled in other aspects of your office setting.

. that this information is to be shared only with the patient or a duly authorized representative of the patient.

. that individuals accessing patient information exercise due care to log off or lock their workstations if they will be leaving them unattended for long periods of time.

. that individuals accessing patient information may not share their logon IDs or passwords and shall exercise due care to safeguard their logon ID and password to ensure no unauthorized persons may log on to the system.

. that you will terminate access capabilities of your employees no longer working for you or whose job functions do not require access, or of those individuals no longer associated with your practice.
